• To support or coordinate a multidisciplinary team of health care professionals within the hospital setting in order to create a smooth transition from hospital to home.
• To provide state-of-the-art, reliable ventilators and ancillary equipment to the home.

• Pre-discharge training by respiratory therapists.
• Home evaluation/electrical check.

• Respiratory therapists on call 24 hours a day.
• Complete line of respiratory and durable medical equipment available
• Infusion therapy along with supplies if needed.
• Ongoing post-discharged follow-up and support to maintain compliance with prescribed home care plan.
• Following discharged facilities protocol for follow-up as well.
